Please complete the following 3 forms and fax or email them to the office at least 48 hours prior to your initial session. The forms can be faxed to (908) 636-2574 or emailed to cjfitz14@aol.com. Doing so would greatly expedite the process of registration, and will maximize the clinical time of the initial session. Please be sure to complete the forms fully as possible. Please note that even if your appointment has been made for you at the time of initial contact, it is not considered confirmed until all 3 forms have been fully completed and received by the office.
